How we use it

We use your information to review your request, contact you, and help identify care or community resources. With your permission, we may share the completed intake with a provider or agency that may be able to help.

Important limits

Never provide a Social Security number, banking information, passwords, or a government ID number. This intake is not an emergency service and does not provide medical advice.
